ACCEPTED INSURANCE:
Most PPO’s; HMO belonging to THIPA Medical Group; Medicare; Cash
CASH PATIENTS:
No insurance. No problem. We have the most competitive cash rate in the South Bay. Look out for monthly specials on office visits and general exams. Call the office for rates.
E-MAIL:
General and billing questions can be submitted via email. Appointment requests can be submitted via email. Medical advice cannot be given via email so if you ask a question regarding medical issues, one of our office staff will call you to make an appointment. You may email us using the form on our contact us page.
We’re glad you have chosen us to provide your medical care, but if you miss your appointments, you compromise that care.
We want to remind you of our office policies regarding canceled and missed appointments. A doctor/patient relationship is built on mutual trust and respect. As such, we strive to be on time for your scheduled appointments, and ask that you give us the courtesy of a call when you are unable to keep your appointment or if you are running late.
CANCELLATION POLICY:
Cancellations should be made 24 hours before your scheduled appointment. If it is after hours, please leave a message with the answering service
MISSED APPOINTMENT POLICY:
A missed appointment is when you fail to show up for an appointment without a phone call, or cancel without at least 24-hour notice.
1st missed appointment will incur a fee of $25.
2nd missed appointment will incur a fee of $50.
PRESCRIPTION POLICY:
Please let your provider and her nurse know if you take your prescriptions to a local pharmacy or use mail order. Also let them know if you get a 30 or 90 day supply of medications. If your prescription is written incorrectly the pharmacy may not fill it and the physician will be asked to rewrite it. If you use a mail order pharmacy, you must mail the original prescription, not a photocopy.
When necessary, prescription refills are ONLY done during our regular business hours. Your pharmacy can fax your refill request to our office. On those rare occasions when a refill by phone is necessary, please be prepared to provide the name of the medication, dose and the pharmacy number. The refill cannot be completed without the information. Allow up to 72 hours for prescription refills to be called to the pharmacy. Always check with the pharmacy before calling the office to see if the refill is complete.
Remember to check the number of refills left on your current annual prescriptions. When you have 2 refills left, it is time to call and schedule your annual exam. We cannot know which medications are covered under which plans, thus, we prescribe based on your needs. If your pharmacist offers a generic replacement, you may accept it. However, some generics will have different side effects than the brand names.
